Due to the outbreak of the novel Coronavirus (COVID-19), FPACC and Westin Hotel are taking extra precautions with the care of every guest to include the possibility of health screening and enhanced sanitation/disinfection procedure in accordance with Seattle and King County Health official guidance.

Symptoms of Covid-19 include:

the above symptoms, as well as all household members, do not currently have, nor have experienced the symptoms listed above.

that I, as well as all household embers, have not knowingly been exposed to anyone diagnosed with COVID-19 in the LAST 30 DAYS.

I understand and affirm;*

that I will report to the appropriate officials if I, or anyone in my household, have been exposed to or diagnosed with COVID-19 prior to the 2022 INTERNATIONAL TRADE AND BUSINESS DEVELOPMENT BI-ANNUAL CONFERENCE.

that FPACC (and FCCPNW) officers, staff members, and volunteers cannot be held liable for any exposure to the COVID-19 virus caused by misinformation on this form, the health history provided by each attendees, or at the event.

that will agree and abide by Westin Hotel policies and regulations in place during the event. I will represent my company and/or business in a polite and professional manner in doing so.
